AUTISM CENTER FINDER

Make your next insurance call easier

A practical checklist for conversations with your health plan and prospective center.

Before you call

Have your plan name, member information and the service you are asking about ready. Keep personal plan details off public forms.

Questions for your health plan

  • Is this specific provider in network for my plan?
  • Is the proposed service covered, and are there eligibility requirements?
  • Is a referral or prior authorization required?
  • What deductible, copayment or coinsurance applies?
  • Are there visit limits, exclusions or out-of-network options?
  • Can you send the information in writing and give me a call reference?

Questions for the center

Ask the center to confirm your exact plan, provide written fee information and explain which approvals it needs before the first visit.

A useful term: prior authorization

Your plan may require approval before a service for it to be covered. Ask your plan which services need this step.
